The following procedure has been agreed with

The procedure is intended to provide practices with a mechanism to report adverse events confidentially to the PCT while still ensuring that where necessary further information can be obtained, clarification sought and the incident validated.

The LMC has agreed that it will act as the intermediary.

Completion of the on line form will generate to the LMC an email containing all the details of the event and the practice. The LMC will check the form to ensure that there is no additional information needed and will then allocate a number to each form.
The LMC will forward to the relevant PCT the details of the incident having first removed any identifiable information. The LMC will not release and practice/patient identifiable information without specific consent, but may if requested by the PCT seek further information on the incident. Provision of any further information will be solely at the discretion of the practice.

The LMC had modelled this approach on the highly successful "UK Confidential Human Factors Incident Reporting Programme", known by the acronym CHIRP, which has contributed since 1982 to the enhancement of flight safety in the UK commercial and general aviation industries, by providing a totally independent confidential (not anonymous) reporting system for all individuals employed in or associated with the industries.

The aim is not to replace existing mandatory reporting systems that are currently in place but to provide a means by which practices are able to raise issues of concern without being identified to their peer group, management, or the Regulatory Authority. Anonymous reports are not normally acted upon, as they cannot be validated.

We believe that the LMC has the confidence and support of GPs, practice staff and the PCTs in being able to provide this important service and hopes that it will encourage openness and an ability to report incidents with confidence that there will be no blame, and contribute to improved patient care.
