Job Summary:
The Advanced Clinical Practitioner (ACP) / Advanced Nurse Practitioner (ANP) will provide expert clinical care to patients, working autonomously within their scope of practice to assess, diagnose, treat, and manage patients presenting with a wide range of clinical conditions.

This role includes a high level of clinical decision-making, prescribing (if qualified), and working collaboratively with the multidisciplinary team to deliver safe, evidence-based, and person-centred care.

Key Responsibilities:
Clinical Practice:
– Undertake autonomous consultations, including assessment, diagnosis, treatment, and management of patients with undifferentiated and undiagnosed conditions.
– Manage both acute and long-term conditions within the scope of practice.
– Prescribe medications appropriately, in line with current regulations (if an independent prescriber).
– Request, interpret, and act on diagnostic investigations such as blood tests, ECGs, and imaging.
– Provide health promotion, disease prevention advice, and patient education.

Leadership & Professional Practice:
– Contribute to the development of clinical protocols and service improvements.
– Act as a clinical resource and mentor to colleagues and students.
– Participate in clinical governance, audit, and quality improvement activities.
– Ensure continuing professional development (CPD) and maintain NMC/HCPC registration and relevant ACP competencies.

Collaboration and Teamwork:
– Work collaboratively with GPs, nurses, healthcare assistants, and administrative staff to support patient flow and service delivery.
– Liaise with external healthcare professionals and agencies to ensure coordinated patient care.
– Participate in multidisciplinary team (MDT) meetings and case reviews.

Administrative Duties:
– Maintain accurate and timely clinical records in line with organisational policies and national guidelines.
– Ensure appropriate documentation of consultations, referrals, and follow-up plans.
– Support the practice in achieving QOF targets, enhanced services, and other performance measures.
