CARE COORDINATOR

15.5 hours per week

Monday 9 – 12.30 and 1 – 6pm
Wednesday 6.30 – 8pm (improved access)
Friday 9 – 12.30 and 1 – 6.15pm

A care coordinator role provides extra time, capacity and expertise to support patients in preparing for or in following-up clinical conversations they have with primary care professionals. They will work closely with GP and other primary care professionals to identify and manage a caseload of identified patients, making sure that appropriate support is made available to them and their carers ensuring that their changing needs are addressed in a safe and efficient way.

Key role of requirements but not exclusive

  • Proactively identifying and working with a cohort of patients to support their personalised care requirements, using the available decision support aids.
  • Bring together all of patients identified care and support needs and explore their options to meet these into a single personalised care and support plan.
  • Help patients to manage their needs, answering their queries and supporting them to make appropriate appointments with the right organisation.
  • Working alongside the PCN social prescriber helping patients take up training and employment and help patients’ access appropriate benefits where eligible.
  • Ensure that patients have good quality information to help them make choices about their care.
  • Support patients with their health and wellbeing, including assisting patients to access their personal health budgets where appropriate.
  • Provide coordination and navigation for people and their carers across health and care services.
  • Support the coordination and delivery of MDTs within the PCN.
