Reach Healthcare is looking for a care coordinator for a 1 year fixed term contract to cover maternity leave.
The focus of this role will be on safeguarding and coordination of care for patients on our vulnerable register, providing a proactive approach to care.
Based in Walderslade, Lordswood and Gillingham, Reach Healthcare is a progressive NHS GP practice delivering high‑quality, comprehensive primary care to a diverse patient community of over 31,000.
