About the Role
We are looking for a enthusiastic forward thinking clinical pharmacist who will work as part of a clinical team including GPs, clinical pharmacist, PCN pharmacists, nurses and allied professionals, supporting the safe, effective, and efficient use of medicines.
This is a fixed term 12 month contract with the possibility of being extended or made a permanent position.
The role offers a varied workload with a strong emphasis on clinical decision-making, patient-facing care, and service development.
Key responsibilities include:
Conducting structured medication reviews and medicines reconciliation
Managing long-term conditions (e.g. hypertension, cholesterol)
Supporting medication optimisation and prescribing safety
Handling acute and repeat prescription queries
Supporting care homes and vulnerable patient groups
Contributing to QOF, IIF, and local enhanced services
Working collaboratively with the wider PCN and community pharmacy teams

About You
We are looking for someone who is:
A qualified pharmacist registered with the GPhC
Ideally experienced in primary care (though not essential)
An independent prescriber
Clinically confident with strong decision-making skills
A team player with excellent communication skills
Motivated to improve patient care and contribute to service development
