Sutton Valence Group Practice are seeking a skilled and compassionate Diabetic Chronic Disease Nurse to join our nursing team. As a Diabetic Chronic Disease Nurse , you will play a vital role in providing quality patient care to the patient on the chronic disease registers. We would also like you to assist in various medical procedures within the treatment room. This is an excellent opportunity for individuals with anatomy knowledge and a passion for diabetic patient care.
Responsibilities:
- Conduct and manage diabetic patient assessments, including taking vital signs, reviewing medical history, and documenting patient information
- Administer medications and treatments as prescribed by physicians
- Assist in medical procedures such as wound care, vaccinations and other treatment room appointments
- Educate patients on health conditions, treatment plans, and preventive measures
- Collaborate with healthcare professionals to develop and implement patient care plans
- Maintain accurate and up-to-date medical records
- Provide emotional support to patients and their families
Requirements:
- A keen interest in chronic disease is a must
- Valid nursing license and certification
- Proven experience as a Practice Nurse
- In-depth knowledge of human anatomy and medical terminology
- Strong communication and interpersonal skills
- Ability to multitask and work in a fast-paced environment
- Excellent attention to detail and organizational skills
- Compassionate nature and ability to provide emotional support to our patients
