Reach Healthcare is a large clinically-led GP practice and its own Primary care network, patient-centred primary care organisation serving communities across Medway. We have a fantastic team consisting of GPs, Allied Health Professionals, Practice Nurses and a wealth of experienced administrative staff who support the clinical team.
We are currently seeking an Independent Prescribing Pharmacist to join our friendly, multidisciplinary team. This is a fantastic opportunity to contribute directly to patient care, leading on safe and effective prescribing, managing chronic conditions, and supporting medication optimisation across our practices.
What You’ll Do
• Conduct patient consultations and prescribe within your clinical competence
• Perform medication reviews and support deprescribing initiatives
• Collaborate closely with GPs, AHPs, nurses, and other professionals to enhance care pathways
• Take a proactive role in managing long-term conditions and promoting antimicrobial stewardship
• Provide training and guidance to colleagues on medicines-related best practice
What We’re Looking For
✔ GPhC-registered Pharmacist with Independent Prescriber qualification
✔ Experience in a GP or primary care setting
✔ Excellent clinical judgement, communication, and consultation skills
✔ Passion for holistic, patient-focused care
Desirable: Experience with EMIS, chronic disease management, and audit participation.
