Key Responsibilities:
- Provide nursing care and support to patients, including chronic disease management, immunisations, and health screenings.
- Providing specialised care for diabetics or respiratory conditions.
- Work collaboratively with GPs and other healthcare professionals to deliver comprehensive patient care.
- Conduct health assessments and develop care plans tailored to individual patient needs.
- Support public health initiatives and health promotion activities.
- Maintain accurate and up-to-date patient records.
Qualifications and Experience:
- Registered Nurse with current NMC registration.
- Experience in a primary care setting is desirable.
- Experience in diabetes or respiratory is desirable.
- Strong communication and interpersonal skills.
