About Us
We are a progressive, patient-focused primary care organisation serving 33,000 patients across multiple Maidstone sites. Our established Pharmacy Team, led by a Senior Pharmacist, works alongside GPs, nurses, AHPs and a dedicated Prescribing Hub that manages repeat prescribing, freeing pharmacists for patient-facing clinical work and proactive medicines optimisation.
The Role
Join us as an experienced Primary Care Clinical Pharmacist combining medicines management with innovative clinical practice. You’ll also work within our GP-led MDT in the Same Day Assessment Hub (SDAC), supporting triage and assessment to deliver safe, timely care.
Key Responsibilities
Structured Medication Reviews (SMRs): polypharmacy, frailty, LTCs
Patient-facing medication reviews (remote & in-person)
Deprescribing and medicines optimisation initiatives
Safe prescribing and medicines management in partnership with the Prescribing Hub
Patient education and adherence support
Prescribing audits, medicines safety and QI projects
SDAC participation: triage assessment within the GP-led MDT
Expert advice on complex prescribing decisions
About You
GPhC-registered Pharmacist
Independent Prescriber (or working towards)
Significant experience in primary care/clinical pharmacy
Excellent consultation, communication and clinical decision-making skills
Able to work autonomously and within an MDT
Full driving licence and willingness to travel across sites
Passion for service development, patient safety and innovation
