HCA

Full Time or Part Time - Medway

Maritime Health Partnership is a progressive primary care provider working ‘at scale’ in Medway and are seeking an additional experienced and patient focused HCA to join our dynamic multidisciplinary team.

Our GP Led MDT enjoys a wide and diverse skill mix, consisting of GP, ANP, Clinical Pharmacist, Specialist Chronic Disease Nurses, Practice Nurses, Complex Care Nurses, Mental Health Nurse, Paramedic, Physio and SPLW supported by a strong, forward thinking management and admin team all working together in delivery of excellent care to our patients.

Job Purpose:

Working as part of the wider Nurse team, the post holder will deliver a structured portfolio of work within their sphere of competency. The HCA will assist our trained nurses and other health care professions in the delivery of key areas of work and the delivery of the highest quality patient care, ensuring a patient centred approach sensitive to the needs of individuals.

Main Responsibilities

To carry out assigned tasks involving direct care of patients, with the support of the nursing team and with appropriate training.

Administrative tasks, and where appropriate training has been undertaken, extended roles.

To follow all practice procedures and protocols.

Administrative

· Maintain contemporaneous patient records

· Maintenance of dry stores and ordering stocks where necessary within practice budget

· Exception report problems with equipment and supplies

· Maintenance of stock balances of vaccines and ordering as required

· Carry out admin to support QOF targets as required

· Carry out any associated administration in connection with audit activities (eg maintaining the phlebotomy spreadsheet up to date)

· Preparation for and support for Nursing Home visits

Clinical

· To deliver new patient health checks

· To provide a phlebotomy service for patients

· To deliver tissue viability/wound management to include dressings and compression bandaging after appropriate assessment by RGN

· Removal of sutures/clips

· Blood pressure checks and monitoring

· Recording ECGs

· Provide general urine testing and recording and specialist tests for glucose and micro albumin

· Weight/height measurement and management

· Assistance at family planning clinic once a month

· Chaperoning, as appropriate

· Infection control checklist including cleaning of any clinical equipment

· New patient health checks

· Venepuncture

· Urine testing/analysis

· Health promotion

· Smoking cessation/Lifestyle advice

· Flu under PSD

· Vaccination (with appropriate training)

Maritime Health Partnership is committed to safeguarding and promoting the welfare of vulnerable adults and children so expects all employees and volunteers to share this commitment as well. Your role will require a DBS check and a yearly declaration or registration with the DBS update service. Cost of DBS application will be met by Maritime Health Partnership.

Pay and Benefits:

Pay Band 3 to 4 (depending on experience)

5 weeks annual leave (plus bank holidays)
