Medway South PCN is a forward thinking and pro-active PCN which prioritises patient care within our community. We have 5 member practices with approximately 61000 patients and have an ambitious team of Management and Additional Roles Staff who aim to achieve a high standard of care working collaboratively with all our member practices.
Our Additional Roles teams consist of
Pharmacists
Physiotherapist
Social Prescribing Link Workers
Health and Wellbeing Coaches
Mental Health Nurses
Paramedics
and a team of Care Co-ordinators aligned to the individual practices
An exciting and challenging new position has been created for a PCN Administration Care Coordinator to work with our Senior Management Team to help the Operations Managers on a daily basis with a variety of administration tasks as detailed in the attached job description. The applicant must have a competent knowledge of Emis Web clinical system, IT skills, MS word and Excel although training will be given to the right candidate.
The role will require visiting the various practices within the PCN and therefore the candidate would ideally be able to drive and hold a valid driving license.
