MPA Group Practices is a well-established Primary Care business and are proud to offer the highest standard of patient-centred healthcare across the five sites covering the Medway Towns and Swale region.

An exciting opportunity has arisen at MPA Group Practices to recruit a new Advanced Nurse Practitioner to join our friendly and supportive practice team at Aylesford Medical Centre.  We are looking to appoint a motivated, enthusiastic and experienced clinical practitioner who is able to work effectively within out clinical team to undertake patient assessment, diagnosis and clinical management.

You will be working closely with salaried GPs ; practice nurses; advanced nurse practitioners; Physician Associate; health care assistants; Podiatrists and Phlebotomists and approximately 33 administration staff including reception. We are friendly, approachable and supportive of our staff and encourage our staff to reach their full potential.

The ideal candidate will have experience of

• working within a nursing team
• ability to autonomously provide clinical assessment
• investigation, treatment and management.
• Ability to review and follow up patients as required.
• Experience in chronic disease management.
• Ability to communicate with patients via a range of media, including face to face, video and telephone consultations, telephone triage, palliative care management and home visits

An understanding of the EMIS Web or similar clinical system is desirable
