We are seeking a dynamic, resilient, Practice Nurse (and/or Practice Nurse requiring further development) to join our growing multidisciplinary team, consisting of GPs, ANP, Clinical Pharmacist, Specialist Chronic Disease Nurses, Paramedic, Mental Health Practitioner and HCA, together with a strong and experienced management and admin team able to support the delivery of excellent care to our patients.
This advert is for vacancies in our Medway (Rochester, Chatham and Gillingham) and Maidstone Medical Practices.
The post holder will be flexible, hardworking and empathetic and responsible for the delivery of practice nursing activities to the practice population. They will work collaboratively with the Nurse Manager and general practice team to meet the needs of patients, supporting the delivery of quality QoF and enhanced services, maintaining safe infection control processes, supporting the delivery of immunisation (both child and adult) and cervical screening programmes, safeguarding, policy and procedures, and providing nurse leadership duties as required.
You will be responsible for ensuring the delivery of safe and effective nursing care to the whole practice population, working closely with the Nurse Manager and Partners to deliver organisational priorities in our Practice locations in Medway and Maidstone.
Clinically, the focus of the role is management of patients with long-term conditions and general nursing interventions for all patients. You will be supported by a strong MDT & clinical team working.
Perform any investigatory procedures requested by clinicians.
Participate in the administrative and professional responsibilities of the practice team, ensure accurate notes of all consultations and treatments are recorded in the patient notes.
Applicants must possess a RGN qualification. We would be open to newly qualified nurses applying for the role.

We are a dynamic and forward-thinking practice with a growing list size of 6 sites and circa 50,000 patients in Medway and Maidstone. We are the Lead Practice for the Primary Care Network and are also about to embark on an ambitious practice development project which will require us to expand our clinical and non-clinical workforce at pace and want to ensure our infrastructure can keep up!
