We are seeking a Health and Wellbeing Coach who is eager to make a difference at Medway PCN.

Medway Central PCN are an innovative and forward-thinking network of 5 GP Practices located in Central Chatham and Gillingham in Kent. With a strong background in training and development of the primary care workforce, the network offers a supportive working environment. Medway Central PCN prides itself on setting the bar high for impacting population health management and as such the current multi-discipline team provides great care to the circa. 50,000 patients within its catchment.

Medway Central PCN includes the following Member Practices:

  • Balmoral Gardens
  • Bryant Street Medical Practice
  • Brompton Medical Centre
  • The Kings Family Practice
  • Maritime Health Partnership

As a Health and Wellbeing Coach Medway PCN you will be a key member of the team, playing a critical role in engaging patients and use health coaching techniques to support them to take an active role in their health and wellbeing.

The post holder will work closely with the other members of the primary care team and complex care teams in the management and decision making about care and service provision for individual patients.

This will include:

  • Care planning, health coaching and delivery of systematic self-management support based on a knowledge of individual’s activation levels.
  • Support effective team working in primary care networks through taking on appropriate practice-based tasks and attending regular multidisciplinary team (MDT) meetings when required.
  • Support for individuals to access appropriate community resources and services

The postholder will have access to appropriate clinical supervision and an appropriate named individual in the PCN to provide general advice and support on a day-to-day basis.

Requirements:

The ideal candidate will have previous experience in health and nutrition work with patients, and experience of working with individuals with long term conditions, but if you have similar experience, and feel your skills are transferable, please do consider applying for the position.

The post holder must the ability to demonstrate sensitivity, empathy and compassion to the needs of the service users and carers, and the ability to deal with challenging behaviour.

Note: Successful candidates are required to provide documented evidence of two doses of Covid-19 vaccination as a condition of deployment. This is a standard requirement for individuals working in a Care Quality Commission (CQC) regulated health or social care workplace unless exempt.
